In this second installment, I cover the anatomy of an episode, showing that post acute costs for common conditions rival that of the index hospitalization. Then I walk through specific steps physicians can take in the course of patient care to succeed in bundled payment programs. I will say that all of the steps represent patient-centered, high-value care. I finish by describing some key health policy shifts toward ‘value based payment’ that can inform the strategy of physician practices in the coming years.
