Industry publication Fierce Healthcare shared Dr. Win Whitcomb’s strategies for post-acute care in an article published on October 3, 2016 that recapped Dr. Whitcomb’s panel at the CAPG Colloquium 2016.

The article shared the four key actions Dr. Whitcomb advised that doctors can take to better direct patients to the appropriate post-acute care.  They are to:

• Order an evaluation to explore the possibility of home health
• Ask “Why not home?” when discussing next site of care
• Consider palliative care options, which may best be administered at home
• Be sure to communicate with home care givers or the post-acute care facility, especially for for high-risk patients
