The Lewin Report identifies several interventions that contribute to success in the BPCI program, including Field Operations Staff, Post Acute Networks, and Preferred Provider Lists, and Technology to facilitate identification of and tracking of BPCI eligible patients. One southern hospital achieved a decrease in average LOS for Network SNFs due to the management of the Post Acute Network relationship by Remedy’s PAN Reps. In Q2 2016, the average LOS for SNFs was 13.7 days compared with 29.7 days for non-Network centers.
Since implementing the SNF Performance Network in early January 2016, utilization of network facilities has increased from 64% to 86%.

The benefits of PAN Rep Network management can also be seen in the case of a California medical center where SNF length-of-stay decreased from 43 days to 13.7 days in Q2 2016. While Network SNF LOS has decreased most rapidly, non-Network centers have caught up, indicating an effort to meet expectations for Network inclusion.
Management of Network relationships by PAN Reps is key to BPCI success in the post-acute environment. Networks developed and monitored by PAN Reps have demonstrated:
- Increased SNF Network Utilization
- Decreased Average SNF LOS Overall
- Lower Average SNF LOS Initially in Network SNFs compared with Non-Network SNFs
- Improvement in non-Network SNFs to meet Network expectations
PAN Reps have a significant role in the successful implementation of BPCI programs. Their key functions are to
- Support SNF and HHA Network selection by collecting qualitative information on local SNFs through surveys, meetings and tours
- Build relationships with Network providers to track patients and manage utilization
- Assist with Episode Connect provisioning and integration
- Establish Patient ID processes
- Receive and review patient status updates and manage against ELOS guidelines/other expectation
- Monitor performance of Network Providers and support improvement
- Provide and analyze data for Network center
- Identify areas of opportunity
- Re-evaluate SNF Network selection approximately every 6 months
